Cancer malnutrition: Feeding everyone from hospital to home aims to partner with primary care and community sectors to better understand education needs in regards to cancer malnutrition through surveys to dietitians, GP’s and practice nurses across the continuum of care, share learnings and target resources to improve nutrition care for patients between hospital and home. This project also seeks to promote the value of cancer malnutrition as a mandatory quality and risk indicator across all health sectors in Victoria.
