The optimisation clinic will operate at Peter MacCallum Cancer Centre with a model of care developed in collaboration with Melbourne Health and primary and community health partners for referral pathways to sub-acute and community based rehabilitation services. The clinic will include a dietitian, psychologist, physiotherapist, occupational therapist with screening and triage of referrals into the clinic to be completed by an allied health assistant.
