Download the Registration form from this website. Complete the Registration form and post or fax to the Department of Nursing Education as outlined on the form. Registration will be confirmed on receipt of registration form. |

Peter MacCallum Cancer Centre is located on the Cnr St. Andrews Place & Lansdowne St. East Melbourne (see Location for map public transport information) The Department of Nursing Education is on Level 3, Smorgon Family Building. |

CERTIFICATES |
|
| |
Certificates are issued to all participants who complete 80% of the course for which they enrol and gain a satisfactory pass in any assessment. Certificates indicate allocation of Royal College of Nursing Continuing Education points. |
|
|
|
CATERING |
|
| |
Fee includes light lunch and morning and afternoon refreshment. |
|

PARKING |
|
| |
Peter Mac does not have a public car park. Wilson Parking provides early bird rates (in before 9 am, out after 3 pm) in the Park Hyatt, adjacent to the hospital. Ph: (03) 9224 0301 |

FEES |

Please note that all Short Course Program Fees include GST Fees can be paid by cash, cheque or credit card (BankCard, MasterCard or Visa only). Cheques should be made payable to Nursing Education, Peter MacCallum Cancer Centre. Fees must be paid two weeks prior to course commencement to ensure registration. |

CANCELLATIONS
& REFUNDS |

Cancellations made up to one (1) week prior to course commencement will be refunded at 80% of the fee. There is no refund for later notification. Peter Mac reserves the right to cancel any course that does not meet the required enrolment. Course fees will be fully refunded in the event of a cancellation by the organiser.. |
