| Moving Forward with Confidence: Increasing the Capacity for Self-Management in Cancer Survivors. |
| This project will focus on the development and implementation of a survivorship care plan, entitled ‘My Health and Wellbeing Plan’. ‘My Health and Wellbeing Plan’ was originally developed by the Queensland University of Technology with the aim of promoting self-management for cancer survivors. The plan includes: a medical treatment summary; a section to identify the survivor’s concerns and hopes; a self-management focussed goal setting component and targeted health and service contact information. Plans will be developed either in a 1:1 session or within a group program. |
| A copy of the plan is provided to the participant, their GP, the Cancer Council Victoria and patient’s health service medical record. Participants will be encouraged to take their plan to their GP and outpatient appointments. |
| Cancer Council Victoria Helpline Nurses will provide telephone support at 4 weeks, 4 months and 8 months following development of the plan. Following each phone intervention, Cancer Council Victoria will send a written update to the participant, their GP and the health service. |
| This project will be piloted across the following services: Eastern Health: Ambulatory Oncology Rehabilitation Program Northern Health: Colorectal Service Austin Health: Haematology Service and Breast Cancer Psycho-educational Transitions Group |
| Health professionals involved in the project will receive education and mentoring in skills to facilitate patient self-management |
| NEMICS is piloting a survivorship care plan entitled ‘My Health and Wellbeing Plan’. ‘My Health and Wellbeing Plan’ was originally designed by Professor Patsy Yates of Queensland University of Technology (Q.U.T.). The plan takes a chronic disease self-management approach to supporting cancer survivors to transition off active hospital-based treatment. NEMICS partners and consumers have developed a goal setting template that has been added to the Q.U.T. plan and have tailored the treatment summary component of the plan for each tumour group involved in the project.
