Surviving Cancer: Model of Care to Improve Health Outcomes Among Survivors of Cancer.

The Project will trial a survivorship program for cancer patients in two regional settings integrating GP and specialists' care. The specific objectives of the Project are to:

- Trial a risk stratified model of care that provides comprehensive and patient centred approach to survivorship;
- Explore the feasibility of accessing MBS funding streams to support a structured program of care for cancer survivors in acute and primary care setting;
- Improve sharing and transfer of knowledge between the acute and primary care providers within two regional centres;
- Develop a locally based primary care community of practice for general practitioners with a special interest in cancer care;
- Identify existing local community service initiatives that could be accessed by cancer survivors;
- Create an infrastructure to support evidence-based practice in survivorship care;
- Develop an effective tool for capturing long term patient outcome data;
- Evaluate the program to determine its effectiveness, acceptability, sustainability and transferability to other regional areas within BSWRICS;
- Contribute to the national and international evidence base on effectiveness of survivorship programs.
