General Information for GP's & Specialists: Referring your patient

This clinic operates on Tuesday mornings, Thursday afternoons and Friday mornings.

Referral Requirements

All new patient referrals will be assessed using a collaborative approach by the Multidisciplinary Team (MDT). All patients must be referred to an individual clinician in the service or the head of the service if you are unsure of the appropriate doctor and your patient will be placed with the correct consultant.

Completed referral form with histopathology X-rays - radiology and nuclear medicine images (if completed) Histopathology if biopsied Operative Reports (if carried out) Please advise if there has been any previous treatment of this lesion Would you consider this patient suitable for excision on the same day as initial appointment.

The urgency and clinic allocation are determined by referral content. Many clinics have all referrals read by Specialist Clinic Consultants and Nurse Coordinators If you are concerned with the appointment time allocated to your patient, please phone the New Patient Registration Clerk 9656 1335 or page the Nurse Coordinator on 9656 1111 pg 7323.

All URGENT REFERRALS must be forwarded directly to the triaging clinician to obtain appropriate prioritisation.

Urgent - likely to receive an appointment ≤ 1 week Soon - likely to receive an appointment within 2-3 weeks Routine - likely to receive an appointment ≥ 3-4 weeks

Please note that the unit is unable to accept routine skin checks and clearly benign skin lesions - these patients will be directed back to their GP or a local dermatologist.
