General Information for GP's & Specialists: Referring your patient



The clinic for this service operates weekly on a Monday morning.

Referral Requirements


All patients must be referred to an individual clinician in the service or the head of the service if you are unsure of the appropriate doctor and your patient will be placed with the correct consultant.

Completed referral form with chest x-rays
Chest and Abdominal CT Scan, Bronchoscopy and Bone Scan, PET Scan (if available)
Sputum cytology reports and other relevant histopathology (if available)
Operation notes (if available)

Send referral and any results or reports by:

• fax to (03) 9656 3743, or

• mail, well in advance, to ensure a timely arrival to:

The urgency and clinic allocation are determined by referral content.

Many clinics have all referrals read by Specialist Clinic Consultants and/ or Nurse Coordinator.

Urgent
- likely to receive an appointment within 7 days

Routine
- likely to receive an appointment within 14 days
