General Information for GP's & Specialists: Referring your patient



This clinic operates weekly on a Thursday afternoon.

Referral Requirements


All new patients are to be referred to Dr Greg Wheeler.

Completed signed referral form with summary of previous treatment:
Histopathology report
  • where was patient previously treated
  • treating clinician if known
  • previous treatment if known
  • Radiation therapy, site and dose
  • Chemotherapy, drugs and dose
  • surgery
  • any known late effects
Diagnostic imaging reports and CD's or films.

Send referral and any results or reports by:

The urgency and clinic allocation are determined by referral content. Many clinics have all referrals read by Medical Consultants and/or Nurse Coordinators. If you are concerned with the appointment time allocated to your patient, please phone the New Patient Registration Clerk on 9656 1335 or page the Nurse Coordinators on 9656 1111, pg 7017 or 7500.

Late Effects Service Referral Guidelines



The Late Effects service at the Peter Mac is designed to prevent, detect, and treat complications as a result of curative cancer treatment. The Late Effects service aims to help people to remain as healthy as possible throughout their lives.

To maximise the physical and mental health of patients after the completion of treatment, medical, nursing and allied health care professionals specialising in the long term follow up will initiate assessments, screening and supports.

The Late Effects service is available to people over 18 years of age who completed cancer treatment at least five years ago and are considered cured.


Referrals


If the referring clinician has any concerns regarding the patient please contact either:

Dr Greg Wheeler or Dr Mary Dwyer on 9656 1111 or the Nurse Coordinators on pager 7017 or 7500.

Patients will be contacted for an initial consultation within 4 weeks of the service receiving a referral. The referral letter should be faxed to 9656 3743 or 9656 1192.
