How to Contact the FCC or Make a Referral

Individuals can access FCC services by either telephoning the Centre directly on (03) 9656 1199 or by referral from a medical practitioner (see referral guidelines below).

Those patients who self refer may be required to obtain a medical referral for an appointment at the Centre at a later date.

Medical practitioners wishing to refer a patient can send a letter of referral directly to the FCC, telephone a referral through on (03) 9656 1199, or send a referral by fax (03) 9656 1539 or email to FamilialCancer@petermac.org.

The FCC provides services from both the Peter MacCallum East Melbourne site and from the Bendigo Base Hospital. Referrals to the Bendigo based clinic are managed through the East Melbourne site. Before attending the clinic, the patient will be asked to complete and return to the centre a family history questionnaire and individual medical history form, this is a vital step as this information is used by the centre to provide an assessment and advice.

There are no out of pocket costs for patients attending the FCC.


Referral Guidelines

These guidelines serve as a guide only. If you are concerned about your/your patient’s personal and/or family history of cancer, please do not hesitate to contact us for advice before referral.

Guidelines:


• Three or more cancers of the same or related types (breast/ovary/uterus/colon/prostate/melanoma/testis) in close relatives on the same side of the family.
• Two cancers of the same or related types in close relatives, at least one diagnosed at age < 50.
• An immediate relative (parent or sibling) with one of the more common adult cancers (breast/colon/ovary/melanoma) at age < 40.
• Clustering of cancers/signs suggestive of a hereditary cancer syndrome, eg. Renal cancer with retinal angiomata, melanoma with dysplastic naevus, multiple paragangliomata, phaeochromocytoma and other MEN-associated tumours (this list is not exhaustive).
• In the same individual or close relatives (eg. Brain tumour, sarcoma < 45 years).
